Participant medical declaration and consent to train. Download, complete and sign the original form before training.

This form records acknowledgement of risk, medical information and consent to train. It does not attempt to waive liability for death or personal injury caused by negligence or any liability that cannot lawfully be excluded.

Participant and guardian details

The downloadable form asks for the participant’s full name, date of birth, age, address, phone or email, and emergency contact information.

A parent or legal guardian must complete the guardian section for participants aged 13–17.

Medical and health information

Please disclose information relevant to safe participation, including breathing or heart conditions, recent injuries or surgery, joint, bone, back or neck problems, seizures or fainting, emergency allergies, relevant medication, skin infections, open wounds and any other relevant condition.

If you answer yes to any item, provide details and seek medical advice where appropriate.

Inherent risks of Jiu-Jitsu training

Brazilian Jiu-Jitsu and submission grappling are physical contact sports. Even when appropriate instruction, supervision, hygiene and safety procedures are followed, participation involves inherent risks, including:

  • Bruising, abrasions, mat burns, cuts and bleeding
  • Muscle strains, tendon injuries, ligament sprains and tears
  • Joint injuries, dislocations and fractures
  • Neck, back, spinal and head injuries, including concussion
  • Accidental pressure or impact to the face or other body parts
  • Falls, trips, throws, being landed on or accidental collision
  • Temporary choking, restricted breathing or loss of balance
  • Skin infections and other communicable illnesses
  • Dehydration, overheating, fainting or exercise-related illness
  • Aggravation of an existing injury or medical condition
  • Rare but serious injury, including permanent disability or death

Participant acknowledgements

  • The information I provide is accurate and complete to the best of my knowledge.
  • I will tell the instructor about relevant injury, illness, medical condition or change in health.
  • I will follow reasonable instructions and stop, modify or seek assistance when instructed.
  • I will use appropriate equipment and clothing and maintain good hygiene.
  • I will not intentionally injure, intimidate or recklessly endanger another participant.
  • I will tap early and clearly and immediately release a submission when my partner taps or the instructor tells me to stop.
  • I may decline a technique or drill if I believe it is unsafe for me and will tell the instructor why.

Emergency medical assistance

If a participant is unable to make decisions in an emergency, the form authorises the Club or instructor to seek appropriate first aid and emergency medical assistance and, where appropriate, contact the participant’s emergency contact.

Liability and legal rights

The document records acknowledgement of inherent risks and agreement to follow safety requirements. It is not intended to exclude or restrict liability for death or personal injury caused by negligence, fraud or fraudulent misrepresentation, or any other liability that cannot lawfully be excluded or restricted. Statutory rights are not affected.

Data protection

Personal and health information provided in the waiver is handled in accordance with the Club’s published Privacy Notice.

Declaration and signatures

The original PDF includes signature fields for the participant and Club representative, plus a separate parent or legal guardian declaration for participants aged 13–17.
